তামাকমুক্ত হাসপাতাল বাস্তবায়ন নির্দেশিকা


তামাক এবং তামাকজাত দ্রব্যের ব্যবহার বিশ্বে হৃদরোগ, ক্যান্সার, বক্ষব্যাধি এবং অন্যান্য অনেক প্রতিরোধযোগ্য রোগের এবং মৃত্যুর কারণ। গবেষণায় দেখা গেছে, তামাক সেবনের ফলে প্রতি বছর বিশ্বে ৬০ লাখ লোক মৃত্যুবরণ করে। বাংলাদেশে প্রায় ৪৩.৩% প্রাপ্তবয়স্ক ব্যক্তি নিয়মিত তামাক সেবন করে এবং প্রতি ১০০ জনে ১০ জন তামাকজনিত রোগে আক্রান্ত হয়।

ধোঁয়াযুক্ত তামাক শুধুমাত্র ধূমপায়ীর স্বাস্থ্যের মারাত্মক ক্ষতি করে না, যারা এর পরোক্ষ শিকার তাদেরও ক্ষতি করে। সিগারেটের ধোঁয়ায় সাত হাজারেরও বেশি ক্ষতিকর রাসায়নিক পদার্থ থাকে যার বেশিরভাগই স্বাস্থ্যের জন্য ক্ষতিকর। ফলে এর প্রভাবে অধূমপায়ীদের মারাত্মক রোগসমূহ হয়ে এমনকি মূত্যূ হয়। পরোক্ষ ধূমপানের ফলে ফুসফুসের ক্যান্সার, হৃদরোগ, শ্বাস- প্রশ্বাসের সমস্যা, স্ট্রোক ও প্রজনন সমস্যা দেখা দিতে পারে। ধূমপান ও তামাকজাত দ্রব্য ব্যবহার (নিয়ন্ত্রণ) আইন, ২০০৫ অনুসারে কিছু কিছু জনসমাগমস্থলে (পাবলিক প্লেসে) ধূমপান নিষিদ্ধ করা হয়েছে। কিন্তু পরিসংখ্যানে দেখা যায়, ৬৯.৪% প্রাপ্তবয়স্ক পুরুষ এবং ২০.৪% প্রাপ্তবয়স্ক মহিলা পরোক্ষ ধূমপানের শিকার হচ্ছে প্রধানত চার ধরনের পাবলিক প্লেসে যথা- রেস্টুরেন্ট (২৭.৬%), যানবাহন (২৬.৩%), হাসপাতাল (৫.৮%) ও সরকারী ভবন (৫.৪%)। ধূমপান ও তামাকজাত দ্রব্য ব্যবহার (নিয়ন্ত্রণ) বিধিমালা, ২০০৬ অনুযায়ী হাসপাতাল পাবলিক প্লেসের অন্তর্ভূক্ত, যা কিনা শতভাগ ধূমপানমুক্ত হতে হবে।

প্রতিদিন দেশের বিভিন্ন প্রান্ত থেকে বিপুল সংখ্যাক রোগী এবং তাদের দর্শনার্থীরা হাসপাতাল বা ক্লিনিকে আসেন। এছাড়া হাসপাতাল গুলুতে চিকিৎসক, সেবিকা, টেকনিশিয়ান সহ অন্যান্য কর্মকর্তা-কর্মচারী মিলে অনেকে চাকুরী করেন। স্বাস্থ্যকর্মীদের মধ্যে ধূমপানের হার কমাতে এবং রোগী ও অন্যান্যদের পরোক্ষ ধূমপানের ক্ষতিকর প্রভাব থেকে বাঁচাতে হাসপাতালগুলো তামাকমুক্ত রাখা জরুরী। তামাকমুক্ত হাসপাতাল- হাসপাতালে কর্মরত চিকিৎসক, কর্মকর্তা-কর্মচারী এবং আগত রোগী, দর্শনার্থী ও অন্যান্যদের মধ্যে তামাকগ্রহন থেকে বিরত থাকার প্রবণতা বাড়ায়, ধূমপানের মাত্রা কমায়, সফলভাবে তামাক নিবৃত্তিকরণের হার বৃদ্ধি করে এবং তামাকের ব্যবহার কমায়। শতভাগ তামাকমুক্ত হাসপাতাল প্রতিষ্ঠা করা যদিও কঠিন, তবুও আশার কথা বাংলাদেশে অনেক হাসপাতাল কর্তৃপক্ষ তামাকমুক্ত হাসপাতাল বাস্তবায়নের কাজ হাতে নিয়েছেন।

হাসপাতালের কর্মকর্তা, কর্মচারী, রোগী, দর্শনার্থী ও অন্যান্যদের তামাকজনিত অসুস্থতা ও মৃত্যু নিয়ন্ত্রণের লক্ষে তামাকমুক্ত হাসপাতাল বাস্তবায়নের জন্য বাংলাদেশ তামাক নিয়ন্ত্রণে চিকিৎসকদের সমন্বয়ে গঠিত United Forum Against Tobacco এর উদ্যোগে তামাকমুক্ত হাসপাতাল বাস্তবায়ন নির্দেশিকা প্রণয়ন করা হয়েছে। এই নির্দেশিকাটির লক্ষ্য-

১. হাসপাতালের কর্মকর্তা, কর্মচারী, রোগী, দর্শনার্থী ও অন্যান্যদের তামাক ব্যবহারের মাত্রা হ্রাস করা।

২. হাসপাতালের কর্মকর্তা, কর্মচারী, রোগী, দর্শনার্থী ও অন্যান্যদের তামাক ব্যবহারের ক্ষতি ও পরোক্ষ ধূমপানের ক্ষতিকর বিষয়ে সচেতনতা এবং জ্ঞান বৃদ্ধি করা

৩. হাসপাতালের কর্মকর্তা, কর্মচারী, রোগী, দর্শনার্থী ও অন্যান্যদের মধ্যে সফলভাবে তামাক নিবৃত্তিকরণের হার বৃদ্ধি করা

এই নির্দেশিকাটি চিকিৎসকদের পেশাজীবী সংগঠনের প্রতিনিধি, স্বাস্থ্য সেবা সংশ্লিষ্ট প্রশাসক এবং অন্যান্য তামাক বিরোধী বিশেষজ্ঞদের মতামতের ভিত্তিতে প্রস্তুত করা হয়েছে।


উদ্দেশ্য

তামাকজনিত অসুস্থতা ও মৃত্যু কমানোর জন্য হাসপাতালের সকল কর্মকর্তা, কর্মচারী, রোগী ও দর্শনার্থীদের জন্য স্বাস্থ্যকর ও তামাকমুক্ত পরিবেশ গড়ে তোলা।


ব্যাপ্তি


এই নির্দেশিকাটি হাসপাতালের সকল কর্মকর্তা, কর্মচারী, রোগী, দর্শনার্থী, স্বেচ্ছাসেবক, চুক্তিবদ্ধ ব্যক্তি বা প্রতিষ্ঠান এবং সংশ্লিষ্ট অন্যান্যদের ক্ষেত্রে প্রযোজ্য হবে। নির্দেশিকাটিতে উল্লেখিত নিয়মাবলী হাসপাতালের অন্তর্ভুক্ত সকল ভবন, হাসপাতাল ভবনে বসবাসকারী কর্মকর্তা-কর্মচারী, বাসস্থান, সামাজিক অনুষ্ঠানাদি সহ সকল দাপ্তরিক কাজের জন্য প্রযোজ্য। এই নির্দেশিকায় হাসপাতালের সকল যানবাহনও অন্তর্ভুক্ত হবে।


শতভাগ তামাকমুক্ত হাসপাতাল বলতে কী বোঝায়?

১। হাসপাতালের সকল স্থানে এবং তৎসংলগ্ন এলাকার তামাক গ্রহন নিষিদ্ধ

২। তামাকসেবী কর্মকর্তা ও কর্মচারী, রোগী এবং দর্শনার্থীদের তামাক পরিত্যাগের পরামর্শ ও সহযোগিতা প্রদান

৩। হাসপাতাল এবং তৎসংলগ্ন এলাকায় তামাক এবং তামাকজাত দ্রব্য বিক্রি বন্ধ করা

৪। হাসপাতাল এলাকায় তামাকের বিজ্ঞাপন অ তামাক কোম্পানীর সহযোগিতায় কোন ধরনের কার্যক্রম বা অনুষ্ঠান না করা

তামাক বলতে ধোঁয়াযুক্ত যেমনঃ সিগারেট ও বিড়ি এবং ধোয়াহীন যেমনঃ জর্দা, সাদাপাতা ও গুল উভয় প্রকার তামাকজাতীয় দ্রব্যকে বোঝায়।


তামাকমুক্ত হাসপাতাল প্রতিষ্ঠার কৌশলসমূহ

তামাকমুক্ত হাসপাতাল প্রতিষ্ঠার জন্য নিন্মের কৌশলসমূহ গ্রহন করা জরুরী-

• হাসপাতাল ভবনের সকল স্থান, হাসপাতাল ভবনে বসবাসকারী কর্মকর্তা-কর্মচারীদের বাসস্থান ও যানবাহন তামাকমুক্ত করা

• হাসপাতালের কর্মকর্তা ও কর্মচারীদের অন্য কোথাও ধূমপান বা তামাক সেবন করার সময় হাসপাতালের নির্ধারিত পোশাক অথবা পরিচয়পত্র ব্যবহার করতে না দেয়া

• তামাকমুক্ত হাসপাতাল করার জন্য প্রয়োজনে হাসপাতালের সকল নির্দেশনা, পদ্ধতি এবং প্রক্রিয়া সংশোধন করা

• হাসপাতালের সকল দাপ্তরিক কাজ ও বিভিন্ন অনুষ্ঠানাবলী তামাকমুক্ত রাখা


তামাকমুক্ত হাসপাতাল বাস্তবায়নের জন্য প্রয়োজন-

১. রোগী এবং কর্মকর্তা- কর্মচারীদের সাথে নিয়মিত তথ্য আদান-প্রদান ও যোগাযোগ

২. স্বাস্থ্য সেবা কেন্দ্রের অবকাঠামোগত উন্নয়ন করা

৩. কর্মকর্তা- কর্মচারীদের প্রশিক্ষণ দেয়া

৪. তামাক নিবৃত্তিকরণে চিকিৎসা সেবা প্রদান করা

উপরোল্লেখিত বিভিন্ন পদক্ষেপের মাধ্যমে এক বছরের মধ্যে তামাকমুক্ত হাসপাতাল প্রতিষ্ঠা করা সম্ভব। তবে পারিপার্শিক অবস্থাভেদে এই সময়কেলের তারতম্য হতে পারে।


তামাকমুক্ত হাসপাতাল বাস্তবায়নের জন্য করণীয়

• তামাকমুক্ত এলাকার সতর্কতা মূলক নোটিশ বা সাইন বোর্ড হাসপাতালের প্রতিটি প্রবেশ পথ ও বাহির হবার পথে সুস্পষ্টভাবে প্রদর্শনের ব্যবস্থা করা। এ ছাড়াও হসাপাতাল ভবনে বিভিন্ন এলাকায় পর্যাপ্ত সংখ্যক সতর্কীকরণ নোটিশ প্রদর্শন করা

• ধূমপান ও তামাকজাত দ্রব্য ব্যবহার (নিয়ন্ত্রণ) বিধিমালা, ২০০৬ এর বিধি অনুসারে ধূমপান নিষিদ্ধ এমন পাবলিক প্লেসসমূহে সতর্কতামূলক নোটিশ প্রদানের সুনির্দিষ্ট বিধান রয়েছে। বিধি ৬(ক) অনুসারে দৃষ্টিযোগ্য স্থানে ধূমপান হতে বিরত থাকা এবং উক্ত স্থানে ধূমপান যে শাস্তিযোগ্য অপরাধ তা উল্লেখ করে সতর্কতা নোটিশ প্রদর্শন করেতে হবে (“ধূমপান হইতে বিরত থাকুন ইহা শাস্তিযোগ্য অপরাধ”)। বিধি ৬(খ) অনুসারে সতর্কতামূলক নোটিশের সর্বনিন্ম আকার ৬০*৩০ সে.মি. এবং বিধি ৬(গ) অনুসারে, তা পাবলিক প্লেসের প্রবেশপথে এবং অভ্যন্তরে এমনভাবে প্রদর্শন করতে হবে যা সবার দৃষ্টিগোচর হয়। তদুপরি বিধি ৬(ঙ) অনুসারে সতর্কতামূলক নোটিশের রং হবে সাদা জমিনে লাল অথবা হলুদ জমিনে কালো অক্ষর। ২০১৩ এর সংশোধিত ধূমপান ও তামাকজাত দ্রব্য ব্যবহার (নিয়ন্ত্রণ) আইনের প্রস্তাবিত বিধিমালা অনুযায়ী সতর্কতামূলক নোটিশের সর্বনিন্ম আকার ৪০*২০ সে.মি. হতে পারে

• একইভাবে হাসপাতালের প্রতিটি যানবাহনে তামাকমুক্ত সতর্কতামূলক নোটিশ বা সাইন প্রদর্শন করা

• হাসপাতালের প্রতিটি ভবনের প্রধান প্রবেশ পথে তামাকমূক্ত হাসপাতালের নীতিমালা প্রদর্শন করা

• প্রতিটি রোগীকে হাসপাতালে ভর্তি ও বহির্বিভাগে অবস্থানের সময় ধূমপান মূক্ত হাসপাতাল নীতিমালা সম্বন্ধে অবহিত করা। সম্ভব হলে ভর্তির পূর্বে ও অ্যাপয়ন্টমেন্টের সময়েও এ সম্পর্কে ধারনা দেয়া ।

• কর্মকর্তা ও কর্মচারীদের নিয়োগের সময় তামাকমুক্ত গাইডলাইন সম্মন্ধে অবহিত করা

• ধূমপায়ী কর্মকর্তা ও কর্মচারীদের বিরতির সময় ব্যতিত হাসপাটাল থেকে ধূমপানের জন্য বাইরে যেতে না দেয়া

• সকল কর্মকর্তা ও কর্মচারী এবং সংশ্লিষ্ট বিভাগকে তামাকমুক্ত হাসপাতাল বাস্তবায়নের জন্য নির্দেশ প্রধান করা

• তামাক বিরোধী বিভিন্ন বার্তা অফিস বিল, প্রেসক্রিপশন, অফিস প্যাড ও অন্যান্য নথিতে ছাপানো

• হাসপাতালকে শতভাগ তামাকমুক্ত করার লক্ষ্যে কর্মকর্তা ও কর্মচারী রোগী ও দর্শনার্থীদের সচেতনতা বৃদ্ধির জন্য বিভিন্ন শিক্ষামূলক কার্যক্রম পরিচালনা করা

• ধূমপায়ী ও বাড়িতে পরোক্ষ ধুমপানের শিকার প্রতিটি রোগী সনাক্ত করা

• হাসপাতালের সকল কর্মকর্তা ও কর্মচারীকে তামাক নিবৃত্তিকরণ সংক্রান্ত প্রশিক্ষণ দেওয়া যাতে তারা রোগী ও দর্শনার্থীদের তামাক পরিত্যাগ করার বিষয়ে পরামর্শ প্রধান করতে পারে

• স্বাস্থ্য সেবা কেন্দ্র এবং তৎসংলগ্ন এলাকায় তামাকজাত দ্রব্য বিক্রি অথবা বিজ্ঞাপন নিষিদ্ধ করা। তামাক কোম্পানী কর্তৃক কোনরূপ সাহায্য-সহযোগিতা পরিহার করা

• হাসপাতালের কর্মকর্তা-কর্মচারী, রোগী অথবা দর্শনার্থীদের তামাক সংক্রান্ত অভিযোগ গ্রহন, লিপিবদ্ধ এবং সংরক্ষণ করা এবং সে অনুযায়ী প্রয়োজনীয় ব্যবস্থা গ্রহন করে সকল অভিযোগ ২৪ ঘন্টার মধ্যে সমাধান করা


তামাক নিবৃত্তিকরণ কার্যক্রম

• হাসপাতালের অভ্যন্তরে একটি তামাক নিবৃত্তিকরণ ক্লিনিক প্রতিষ্ঠা করা

• এই ক্লিনিকের সেবা হাসপাতালের সকল কর্মকর্তা ও কর্মচারী, রোগী এবং তাদের পরিবারের সদস্যদের জন্য বিনামূল্যে প্রদান করা

• রোগীদের প্রতিবার পরামর্শ দেয়ার সময় তামাক বিরোধী বার্তা প্রদান করা

• চিকিৎসক ও সেবিকাদের তামাক নিবৃত্তিকরণ সংক্রান্ত যথাযথ প্রশিক্ষণ দেয়া

• রোগীদের এই ক্লিনিকে আসার জন্য সঠিক রেফারেল প্রক্রিয়া প্রতিষ্ঠা করা। ধূমপায়ী রোগী ও দর্শনার্থীদের মধ্যে যারা তামাক সেবন করতে চান তাদেরকে তামাক নিবৃত্তিকরণ ক্লিনিকের সাহায্য নেয়ার জন্য রেফার করা

• হাসপাতালের ধূমপায়ী কর্মকর্তা ও কর্মচারী যারা তামাক পরিত্যাগ করতে চান তাদেরকে তামাক নিবৃত্তিকরণ ক্লিনিকে আসার জন্য উৎসাহিত করা

• তামাক নিবৃত্তিকরণ ক্লিনিকের সহায়তা দেয়ার জন্য সার্বক্ষণিক ফোন নাম্বারের ব্যবস্থা করা


বিভিন্ন পর্যায়ের স্বাস্থ্য সেবাদানকারীগণের ভূমিকা

তামাকমুক্ত হাসপাতাল প্রতিষ্ঠার জন্য পরিকল্পনা, সময় এবং সদিচ্ছা থাকা প্রয়োজন। তামাকমুক্ত নীতিমালা বাস্তবায়নে স্বাস্থ্য সেবাদানকরী প্রতিষ্ঠানসমুহের পরিচালক, ব্যবস্থাপক এবং চিকিৎসকসহ অন্যান্য স্বাস্থ্যকর্মীদের দায়িত্ব অপরিসীম। তামাকমুক্ত হাসপাতাল বাস্তবায়নের জন্য বিভিন্ন হাসপাতাল ভিন্ন ভিন্ন পদ্ধতি অবলম্বন করতে পারে।

স্বাস্থ্য ও পরিবার কল্যাণ মন্ত্রণালয় বাংলাদেশের জাতীয় স্বাস্থ্য নীতি ও স্বাস্থ্য সেবা সংক্রান্ত কার্যক্রম বাস্তবায়নের জন্য নির্বাহী দায়িত্ব প্রাপ্ত। বাংলাদেশে সরকারী ও বেসরকারী সকল প্রকার স্বাস্থ্য সেবা কেন্দ্রের পরিচালনা নীতিমালা প্রণয়ন এবং বাস্তবায়নে স্বাস্থ্য অধিদপ্তরেরে ভূমিকা অন্যতম। স্বাস্থ্য অধিদপ্তরের আওতাধীন ইউনিয়ন পর্যায় থেকে জাতীয় পর্যায় পর্যন্ত বিভিন্ন স্তরের চিকিৎসা কেন্দ্র সহ বিভিন্ন ইনস্টিটিউট, শিক্ষা প্রতিষ্ঠান অন্যান্য বিশেষায়িত প্রতিষ্ঠান রয়েছে। সুতরাং সকল পর্যায়ের স্বাস্থ্য সেবা কেন্দ্র তামাকমুক্ত করতে স্বাস্থ্য ও পরিবার কল্যান মন্ত্রণালয় ও স্বাস্থ্য অধিদপ্তর অগ্রণী ভূমিকা পালন করতে পারে। এছাড়া বিভাগীয় পর্যায়ে একজন বিভাগীয় পরিচালক (স্বাস্থ্য) এবং প্রত্যেক জেলায় সিভিল সার্জন ও হাসপাতাল পরিচালক বা সুপারিন্টেন্ডেন্ট এবং উপজেলা স্বাস্থ্য কমপ্লেক্সে উপজেলা স্বাস্থ্য ও পরিবার পরিকল্পনা কর্মকর্তা এ ব্যাপারে ভূমিকা পালন করতে পারেন। ইউনিয়ন পর্যায়ে তিন ধরনের স্বাস্থ্য সুবিধা বিদ্যমান- গ্রামীণ স্বাস্থ্য কেন্দ্র, ইউনিয়ন স্বাস্থ্য ও পরিবার কল্যাণ কেন্দ্র এবং ওয়ার্ড পর্যায়ে কমিউনিটি ক্লিনিক বিদ্যমান। এ সমস্ত কেন্দ্রগুলোকেও তামাকমুক্ত রাখতে সংশ্লিষ্ট প্রধানগণ ভূমিকা পালন করতে পারেন।

স্বাস্থ্য অধিদপ্তরের ভূমিকা


• দেশের সকল হাসপাতালকে শতভাগ তামাকমুক্ত হাসপাতাল হিসেবে গড়ে তোলার জন্য নীতিমালা প্রণয়ন ও বাস্তবায়নে সহায়তা করা

• স্বাস্থ্য সেবা কেন্দ্রের কর্মকর্তা-কর্মচারীদের তামাকমুক্ত হাসপাতাল

• স্বাস্থ্য সেবা কেন্দ্রে কর্মকর্তা-কর্মচারীদের তামাকমুক্ত হাসপাতাল গাইডলাইন ওপর প্রশিক্ষণ প্রদান করা যাতে দেশের তৃণমুল পর্যায় পর্যন্ত তারা ভূমিকা রাখতে পারেন

• স্বাস্থ্য মন্ত্রণালয় / স্বাস্থ্য অধিদপ্তর/হাসপাতাল অন্যান্য স্বাস্থ্য কেন্দ্রের সাথে সংশ্লিষ্ট বাণিজ্যিক প্রতিষ্ঠানসমূহের তামাক কোম্পানীর সাথে সংশ্লিষ্টতার ধরন সম্পকির্ত তথ্যাদি সুস্পষ্ট নীতিমালার আওতায় প্রকাশ করা

• চাকরীতে নিয়োগের খেত্রে অধুমপায়ীদের অগ্রাধিকার দেয়া


হাসপাতাল এর পরিচালক এর ভূমিকা

• হাসপাতালে সমন্বিত তামাকমুক্ত নীতিমালা বাস্তবায়নে উদ্যেগী হওয়া এবং অগ্রণী ভূমিকা পালন করা

• তামাক নিয়ন্ত্রণ বিষয় সমূহ সকল স্বাস্থ্য সম্পর্কিত সেমিনার এবং কনফারেন্সে অন্তর্ভূক্ত করা। আঞ্চলিক এবং জাতীয় পর্যায়ে তামাক নিয়ন্ত্রণ এর ওপর কর্মশালার আয়োজন করা এবং এ ধরনের অনুষ্ঠানে অংশগ্রহন করা

• হাসপাতালের অভ্যন্তরে তামাক নিবৃত্তিকরণ ক্লিনিক স্থাপনে উদ্যেগ নেয়া এবং এই কেন্দ্রে চিকিৎসা নেয়া রোগীদের ইতিহাস রেকর্ড আকারে সংরক্ষণে প্রয়োজনীয় ব্যবস্থা নেয়া

• তামাকমুক্ত সর্তকতামুলক নোটিশ হাসপাতালের সঠিক জায়গায় প্রদর্শনের ব্যবস্থা করা

• হাসপাতালে চিকিৎসা নিতে আসা সকল রোগীদের তামাক ব্যবহার সঙ্ক্রান্ত তথ্যাদি সঠিকভাবে লিপিবদ্ধ করার ব্যাপারে প্রয়োজনীয় ব্যবস্থা নেয়া

• চাকরীতে নিয়োগের ক্ষেত্রে অধূমপায়ীদের অগ্রাধিকার দেয়া


চিকিৎসক ও অন্যান্য স্বাস্থকর্মীর ভূমিকা

• সরকারী ও বেসরকারী পর্যায়ে তামাক নিবৃত্তিকরণ ক্লিনিক স্থাপনের জন্য পদক্ষেপ নেয়া

Tobacco use is a major preventable cause of premature death and diseases worldwide. It is the single common risk factor for many types of cancer, cardiovascular diseases, chronic respiratory and many other diseases. currently, more than 6 million people worldwide die each year due to tobacco related illness.l In Bangladesh about 43.3% adult use tobacco and about 10 out of 100 people suffered from one of the eight identified tobacco-related diseases. 2, 3

Smoking tobacco not only increases risk of serious illness to the individual smoking, but also to other exposed to second hand smoke (SHS). 4 SHS is a mixture of more than seven thousands of chemicals, most of which are harmful and cause serious disease and death in the non-smoking population. Secondhand smoke is a known cause of lung cancer, heart disease, low birth-weight births, and chronic lung ailments such as bronchitis, as well as other health problems. In Bangladesh smoking in public places is prohibited by law. Smoking and Tobacco Products Usage (Control) Act, 2005 recognized hospital as "Public Place" which should be 100% smoke free. However it is reported that 69.40% of the male and 20.8% of the female adult population are exposed to SHS in public places and among the four main categories of public places, people are exposed in restaurants (27.6%) and public transportation (26.3%), health-care facilities (5.8%) and government buildings (5.4%).2

Everyday large number patients and their attendants visit hospitals or clinics at different parts of Bangladesh. Hospitals also employ large number of staff which includes physicians, nurses, technicians and other administrative and support staff for providing service. Keeping hospitals tobacco-free is crucial to reducing smoking rates among health workers and eliminating the exposure of patients and staff to secondhand smoke. Tobacco free policies reduce consumption by smokers, increase smoking cessation attempts, increase the number of smokers who successfully quit, and reduce the prevalence of tobacco use among workers. Effective of smoke free policies reduce exposure.4

Despite the challenges of managing a l00% smoke-free hospital property, many administrators of hospitals in Bangladesh have started to act for implementation of tobacco free hospital campus. A guideline for making the hospital campuses tobacco free taking account of the local context will help the hospital administrators to implement the tobacco free policy in hospitals.

United Forum against Tobacco (UFAT), a physician's forum for tobacco control in Bangladesh has taken the initiative to develop Juch a guideline as part of a toolkit to promote and support tobacco-free policies in all healthcare facilities

with the aim of reducing tobacco-related morbidity and mortality among the staff, patients and broader communities and to specifically:

l. Reduce exposure to tobacco smoke among staff, patients, visitors and the broader community.

2. Increase awareness and knowledge ofthe harm caused by tobacco use and exposure to tobacco smoke among staff, patients and the broader community.

3. Increase the number of staff, patients/family members, and the broader community who successfully quit using tobacco.


The guideline provides guidance to healthcare facility senior management teams, administrators, health professionals, communication experts and other key stakeholders in developing a tobacco-free healthcare facility policy.


What is a l00% tobacco -free facility?

l. No smoking is allowed on the healthcare facility premises, with smoking banned in the grounds or designated smoking areas outside.

2. All staff and patients who use tobacco, and family members who expose patients to smoke in the home, are offered support to quit.

3. No tobacco products can be sold on the healthcare facility premises and areas outside nearby.

4. No tobacco advertising or sponsorship is allowed on the premises.

Tobacco includes both smoking such as cigarettes, biri and smokeless such zarda, gul, sadapata etc


Purpose

To provide a healthy, tobacco-free environment for all staff, patients and visitors, and to reduce tobacco-related morbidity and mortality among staff, patients and the broader community


Scope

This policy applies to all (healthcare facility) staff, patients, visitors, volunteers, contractors and others accessing the facility. It applies to all buildings and grounds owned or occupied by (healthcare facility) including residential accommodation, and business and social events. It applies to all vehicles owned by the (healthcare facility).


Model 100% tobacco-free healthcare facility

A best practice models has been developed which is focused on two outcomes:

1. A 100% tobacco-free healthcare facility with smoking banned in the grounds or designated smoking areas outside.

2. Accessible, affordable, and available tobacco cessation support for all staff and patients who use tobacco, and/or family members who expose patients to tobacco smoke in the home.


Strategies for establishment of tobacco Tobacco-free hospital

A tobacco free policy of the hospital need to incorporate following strategies -

• All the (healthcare facility) buildings (including staff residences), grounds (may include designated smoking areas), ofEces and vehicles must be declared tobacco-free.

• Staffs, who wish to smoke off-site, should not be identifiable as the (healthcare facility) staff by their uniforms or name tags.

• All the (healthcare facility) policies, processes and systems will be amended to reflect the requirements of this policy.

• All the (healthcare facility) business and social functions will be nonsmoking


Following tools can be utilized for implementation of tobacco free hospital policy.


1. Inforrnation and communication with patient and staff

2. Development of a system

3. Stafftraining

4. Cessation service

Ideal time frame required for establishing a 100% tobacco free hospital is about one year however depending upon the commitment and resource allocation by authority the time frame may vary.


Actions to be taken for implementation of tobacco free hospital policy

• No smoking signs will be clearly visible at all entrances and exit points throughout the (healthcare facility).

• According to the smoking and using of tobacco products (Control) rules 2006 of Bangladesh, hospital authorizes should display the warning notice at the entrance and other clearly visible areas of the buildings. The notices shall be at least 60cm x 30 cm, written in red letters with white background or in yellow letters with black background. The notice should be in Bangla and in necessary in English and should state " Be abstain from smoking, It is punishable offence". The proposed rules of the amended act of 2013 suggested minimum size of notices as 40 x 20 cm

• No Smoking signage for vehicles.

• The tobacco-free policy will be displayed at the main entrance of every building.

• Patients will be informed of the tobacco-free policy at the time of admission or outpatient appointments. Elective service appointment letters will include reference to the tobacco-free policy.

• Staffwill be informed of the tobacco-free policy at recruitment and orientation.

• Staff who wish to leave the (healthcare facility) to smoke may only do so during designated meal/tea breaks.

• An order could be circulated for all staffs and related department for implement the policy.

• Smoke free messages can be printed in all bill, prescription, official pad, letter, circulations and all other documents.

• Before the 100% enforcement of the policy, there will be targeted and intensive educational campaigns and promotional events to popularize the tobacco free policy like orientation, session for patients or staffs, any IEC materials etc.

• All patients who use tobacco or ilre exposed to tobacco smoke in the home are identified.

• All health staffs are trained to give brief advice to quit using tobacco to patients who use tobacco and/or family members who expose them to tobacco smoke in the home.

• Tobacco products must not be sold or advertised on (healthcare facility) premises. The (healthcare facility) will not seek or accept sponsorship from any tobacco manufacfurer or company.

• Complaints from staff, patients and/or visitors can be made to (name persorn/contact details) in a complain register book and preserve it. A11 complaints will be addressed within 24 hours.

• A11 patients who use tobacco, and/or family members who expose patients to smoke in the home are offcred a referral to the (healthcare facility) cessation senrice.


Cessation Service

• A cessation clinic should be established.

• The cessation service is offered to staff, patients and family members' free-of-charge.

• Patients should receive anti tobacco message at each consultation with documentation.

• Physicians and nurses should receive appropriate training for counseling.

• Appropriate referral system for patients to the clinic need to be established

• Staff who use tobacco and wish to quit can self-refer to the (healthcare facility) cessation service.

• Patients who use tobacco, and/or family members who expose them to tobacco smoke in the home can be referred to the (healthcare facility) cessation service.

• A help line or quit line (phone number) could be installed.


Roles of different health care professionals at various levels:

Moving to a tobacco-free environment takes planning, time, and incremental, implementation. Imolementation of the policy and the guidelines is delegated to Directors, Senior Managers and Team Leaders across the entire health system. Not every hospital or health system will start their tobacco-free efforts the same way, or in the same place. T

The Directorate General of Health Services (DGHS) under the Ministry of Health & Family Welfare (MOHFW) is the key agency to implement the national health policies and programs. DGHS also provides technical guidance to the ministry. The role of MOHFW and DGHS has the crucial leadership role in making the hospitals of different tiers tobacco free.6

In each division, divisional director for health and in dishicts health civil surgeon (CS), Superintendent and in upazila health complex Upazlla Health and Family Planning Officer will be responsible for making the hospitals tobacco free. In the union level, one or other of the three kinds of health facilities may exist, viz. rural health center, union sub-center or union health & family welfare center (WHHFWC) and in the ward level, community clinics (CC), the tobacco free policies need to implement by responsible officers of the centers.


Directorate General (Health)

• Promote and facilitate the implementation the tobacc free hospital guideline to all the hospital in the country.

• Orient the smoke free hospital guideline among the officers and staffs of health service.

• Trained officers and staffs for the development of skilled personal in the tobacco control all over the country extending as low as up to village level.

• Discourage recruitment of smoker

• Establish a stated policy on any commercial or other kind or relationship with partners who interact with or have interests in the tobacco industry through a declaration of interest.


Hospital Directors

• Promote and facilitate the implementation of comprehensive tobacco free policies in hospitals and health care facilities based on evidence based approaches and good practice.

• Include tobacco control in the agenda of all relevant health-related seminar and conferences and reach out to organize or participate in tobacco control activities/campaigns regionally or nationally.

• Promote exclusively evidence based tobacco cessation treatment methods that conform to the differing needs of individual tobacco users taking into account socioeconomic status, age, education, ethnicity and gender.

• Check the anti tobacco signage kept in the appropriate area.

• Commit health professionals within the organization to routinely record and monitor the tobacco consumption/smoking status of all patients and record their subsequent actions /interventions in an integrated routine record system.

• Refrain from accepting any kind of tobacco industry support- financial or otherwise and from investing in the tobacco industry.

• Request local civil administration to conduct mobile courts for punishing the violators

• Maintain liaison with local anti tobacco group for concerted efforts.


Health professional

• Take measures to promote smoking cessation therapy with related stakeholders.

• Support the inclusion of tobacco control in the agenda of all relevant health-related seminar and conferences and make sure that all events are tobacco/smoke free.

• Encourage and support staff and colleagues to become role models by not using tobacco and by implementing a tobacco-free culture in the workplace

Annex 1. Structure of Health Care System under DGHS, MOHFW

Health care service in Bangladesh is mainly provided by government hospitals and clinics situated in large cities as well as at union level. Non government organizations and foundations and private sector also provide health care service mainly in urban areas. The Directorate General of Health Services (DGHS) is the largest executing authority under the Ministry of Health & Family Welfare in Bangladesh. Having over one hundred thousand officers and staffs, it operates then health care delivery system for the ministry all over the country extending as low as up to village level. Type of health facilities under DGHS in different administrative tiers are shown in Tabte 1.
